ELIGIBILITY AND ENROLLLMENT
Who is eligible to participate in the plan?
You are eligible to participate in the plan when you have reached age 21 or
older and have completed 1 year of service with a minimum of 1000 hours
worked.
When can I begin contributing to the Plan?
If you are an eligible employee, you can join the Plan and start contributing
on the following entry dates: January 1st and July 1st.
Are rollovers accepted?
PLAN HIGHLIGHTS
If you want to roll over qualified retirement plan assets, you may do so at
any time.
How do I enroll in the Plan?
You can enroll via the website www.CertifiedDifference.com by using the web
login instructions found on page 9 or by completing the forms located within
this book and returning them to your human resources representative.
