ELIGIBILITY AND ENROLLLMENT
Who is eligible to participate in the plan?
You will be eligible for the plan when you have reached age 19 and have
completed 500 hours of Service in a 6-month period. If the service
requirement is not met in the first consecutive period of months, you will also
be eligible for the Plan if you complete one (1) Year of Eligibility service,
provided that you are an Eligible Employee at the end of that period.
When can I begin contributing to the Plan?
You can begin contributing to the plan on the 1st day of the plan quarter
coincident with or next following the time you meet the eligibility criteria
specified above.
PLAN HIGHLIGHTS
You can increase, decrease or stop your deferral contribution as of each pay
period.
Are rollovers accepted?
If you want to roll over qualified retirement plan assets into the plan from a
previous retirement account, you may do so at any time.
How do I enroll in the Plan?
You can enroll via the website www.CertifiedDifference.com by using the
web login instructions found on page 9 or by completing the forms located
within this book and returning them to your resources representative.
