ELIGIBILITY AND ENROLLLMENT
Who is eligible to participate in the plan?
You are eligible to participate in the plan when you have reached age 21 or
older and have completed 1 Year of Service with a minimum of 1000 hours
worked.
When can I begin contributing to the Plan?
You can begin contributing to the Plan on the first day of the month
coincident with or next following the date that the eligibility requirements are
met.
Are rollovers accepted?
If you want to roll over qualified retirement plan assets, you may do so once
you are eligible to participate in the plan.
How do I enroll in the Plan?
You can enroll via the website www.CertifiedDifference.com by using the web
login instructions found on page 9 or by completing the forms located within
this book and returning them to your resources representative.
