ELIGIBILITY AND ENROLLLMENT
Who is eligible to participate in the plan?
You are eligible to participate in the plan when you have reached age 21 or
older and have completed 12 months of service.
When can I begin contributing to the Plan?
You can begin contributing to the Plan on the January 1st or July 1st of the
Plan Year next following the date you meet the eligibility requirements.
Are rollovers accepted?
PLAN HIGHLIGHTS
The Plan may accept a Rollover Contribution made on behalf of any
Employee not excluded from the Plan, regardless of whether such
Employee has met the age and service requirements of the Plan.
How do I enroll in the Plan?
You can enroll via the website www.CertifiedDifference.com by using the
web login instructions found on page 9 or by completing the forms located
within this book and returning them to your resources representative.
