ELIGIBILITY AND ENROLLLMENT
Who is eligible to participate in the plan?
You are eligible to participate in the plan when you have reached age 21 or
older and have completed 1 year of service.
When can I begin contributing to the Plan?
If you are an eligible employee, you can begin contributing on the following
entry dates: October 1st and April 1st.
Are rollovers accepted?
If you want to roll over qualified retirement plan assets, you may do so at
any time.

You can enroll via the website www.CertifiedDifference.com by using the web
login instructions found on page 9 or by completing the forms located within
this book and returning them to your resources representative.
