ELIGIBILITY AND ENROLLLMENT
Who is eligible to participate in the plan?
You are excluded from the Plan if you are a member of any of the following
classes of employees:
* Employees covered by a collective bargaining agreement, for purposes of
Elective Deferral Contributions, Safe Harbor Matching Contributions and
Non-Elective Contributions.
* Non-resident aliens, for purposes of Elective Deferral Contributions and
Safe Harbor Matching Contributions.
You can join the plan when you have attained age 18 and completed 2
months of service.
PLAN HIGHLIGHTS
When can I begin contributing to the Plan?
You may enter the Plan on the date you meet the eligibility criteria specified
above.
Are rollovers accepted?
The Plan may accept a Rollover Contribution made on behalf of any
Employee not excluded from the Plan, regardless of whether such employee
has met the age and service requirements of the plan.
How do I enroll in the Plan?
You can enroll via the website www.CertifiedDifference.com by using the
web login instructions found on page 9 or by completing the form in this
book and returning it to your human resources representative.
